A highly metastatic tumor of endothelial cell precursors which spreads hematogenous and intra-abdominal implantation routes, Spleen is most common primary location and liver is fourth.

A 6y old bernese mountain female admitted to our hospital with weakness, lethargy, weight loss, signs of dehydration and jaundice.

Physical ex: Revealed icteric m.m, arrhythmias, tachypnea, abdominal distension with palpable cranial abdominal mass.

Diagnostic Imaging: Ultras. showed large hypoechoic mass in the spleen and peritoneal effusions.

Laboratory Findings:
-CBC showed marked fall in the HCT and HGB indicate a microcytic hypochromic anemia *anemia of chronic diseases* with severe leukopenia.
-Serum biochemistry revealed elevated ALT, AST, Hypoalbuminemia due to ptn loss in the peritoneal effusions, raised TBIL and DBIL.

Which directed the diagnosis to splenic mass with liver involvement and directed the decision to do splenectomy.

Surgical approach: SPLENECTOMY Providing pre-op blood transfusion for stabilization and compensate the blood loss.
In dorsal recumbency with performing Cr. ventral midline laparotomy by locating the spleen and do ligatures to bl.vs. between the spleen hilus and greater curvature of the stomach splenectomy done successfully and the suspected mass had sent for histopathological ex.

Histopathology Findings: Anaplastic mesenchymal cells arranged in chords forming vascular channels and spaces filled with red blood cells, thrombi, necrotic debris.

Final Diagnosis: Splenic hemangiosarcoma.
